If you are not able to print these forms in advance, a blank set will be provided for you on the day of your appointment. Having forms filled out in advance will increase the time spent doing CranioSacral Therapy.  Contact Emily with any questions. Her cell number and email address are listed above.

Please note that the last form listed (FYI HIPAA Notice of Privacy Practices) is just FYI, it does not need to be printed and signed.
