Please print the following forms for your upcoming CranioSacral Therapy appointment in Skokie:   Intake form Teen Skokie age 12-17, HIPAA Consent, Skokie COVID questionnaire, CranioSacral Therapy Informed consent.

If you are not able to print these forms in advance, a blank set will be provided for you on the day of your appointment. Having your forms filled out in advance will increase the time spent doing CranioSacral Therapy.  Contact Emily with any questions.

Please note that the last form listed (FYI HIPAA Notice of Privacy Practices) is just FYI, it does not need to be printed and signed.
